Part #1: Student Profile Information
This section collects essential student and parent information, including names, addresses, and contact details.
To be filled ONLY ONCE. Move on to next section if this is already done, unless there are any changes.
There is a confirmation message at the bottom of the screen after your submission, please don’t keep filling this more than once.
Part #2: Experience and Skillset
Here, you’ll provide details about the student’s experiences, activities, achievements, and awards. You’ll also upload required documents: a transcript, payment receipt, and a consent form signed by both the student and a parent.
Transcript and consent forms should have your names in the filename for easy identification

Part#3: Essays
You’ll need to submit one set of essays for each project you’re applying for.
The “Project Name” field should exactly match the name of an active SPARK SMP project currently listed online in the Mentorship Project page
You only need to complete Parts 1 and 2 once unless you need to make changes.
